Complex Care Coordinator

Position Details:

Serves a valuable role as part of the Complex Care Management team.
Requires excellent communication skills, an ability to connect with members telephonically and build relationship

Telephonically conducts specially designed  non-clinical Coordinator assessments and records in Centers Plan for Healthy Living (CPHL) electronic care management system
Serves as a friendly support to members and a non-clinical support to Care Manager

Immediately refers any health issue, questions or concerns identified in conversation with member directly to the Complex Care Manager
Participates in Complex Care Department staff meetings as required
 

 

Education and Experience:

 

HS Diploma, Bachelor’s Degree preferred
2 years customer service experience
Experience working within a managed care environment and with the geriatric population a plus
Strong written and verbal communication skills
Microsoft Office Proficiency (Work, Outlook, Excel)
Strong multi-tasking abilities
